Dear Parents or Guardians:

Students entering 5th-12th grades who will be participating in sports must have a current sport physical. 

Students entering the 6th and 9th grades can use the required school physical form.

Physicals must be current in order for your athlete to participate in any sport:

• HS football

• HS cheerleading

• JH/HS basketball

• JH/HS baseball

• JH/HS softball

• JH/HS volleyball

• JH/HS track

• JH Cross Country

Physicals must be on file before the practices begin.  If you have not scheduled an appointment with your primary care physician yet, please do so soon as their schedules fill up quickly.
